To become a Siren7/Siren14/G.719 licensee, please:

1. Print out and carefully read the license below. If you are willing to agree with its terms,
2. Send 2 originals filled in with your legal name and address.
3. Be sure to send the complete contracts of 13 pages including appendices Exhibit A and Exhibit B.
4. Sign and date them (page 9).
